This was a pretty standart caption, can't really think of much of interest to say about it.
Sadly it was the first time I was unable to find the proper source if the image though, so I just refer to the Gelbooru upload where I found it, and will add a link to the artist's profile on whatever platform they use from now on.
Looking through the artist's platforms was pretty interesting though, turns out they made some of the key visuals for Redo Of Healer!
Tim yawns as he wakes up. It takes a few seconds for his eyes to become somewhat adjusted to the darkness, and he notices that none of his friends he had gone camping with were awake yet.
Using the light button, he quickly checks the time on his watch. It was 04:48, the others wouldn't be up for a while, and certainly wouldn't appreciate being woken up yet.
He is well awake now though, and can feel that he won't be able to go back to sleep for a little while. Carefully, he crawls out of his sleeping bag, and sneaks out of the tent.
Finally able to stand straight, he stretches, then takes one of the flashlights from the entrance of the tent.
Not turning the flashlight on yet, he smiles a he looks at the sky. The clearing they'd set up camp at had a great view of the sky, and the stars were beautiful.
The forest they were in was pretty far away from any cities and other major sources of artificial light. The great view of the night sky was a large part of why they chose this location.
As he looks around the campsite, he notices something odd though. He sees a strange, faintly purple glow just outside the clearing. Curious about its origin, he begins to walk towards it.
Since the ground is already slightly lit by the moon and the mysterious light, he doesn't turn on his flashlight yet.
Only a few metres away from the clearing, he finds the source. A small pond, filled with a liquid emanating a soft glow. Trying to get a good look he crouches down as close to it as possible..
"What the?.. ... Ah-woah!!"
Nearly slipping on a patch of moss, Tim jumps up in a panic, loses his balance, and falls forwards into the cold pond.
It's not very deep, and he is easily able to push himself up enough to get his face above the surface, but he feels a strange tingling quickly spreading through his body. Gasping for air, he crawls out of the pond as quickly as possible.
He coughs up some of the water, and brushes his wet hair out of his face allowing him to somewhat see what was happening to his body.
At first he thinks it's just the water making his skin look shinier, but he quickly realises his arms and hands are much more slender and smooth then they were supposed to be.
"Huh..? What the hell is that stuff?.."
Looking back at the pond, he sees it quickly losing its glow. Feeling unsafe, he briskly begins to walk back to the camp.
All the while he feels his bones shifting and his weight redistributing. He almost trips as his hips suddenly pop outwards, almost doubling in width and forcing him to change the way he walks.
"A-ahh!~"
She hadn't noticed when her dick had started to shrink, but the moment it vanished completely, she felt a deep sting in her lower abdomen.

She shakes her head, blushing brightly. Instead she takes the fire starting tools, and quickly relights the fire from last evening, taking care not to get any of the water onto it. Once she is certain the fire won't just go out suddenly, she begins to undress.

After a few minutes, her peaceful basking in the fire's warm light is interrupted though as she hears some rustling inside the tent. Then as she looks over her shoulder, Dan steps out of the tent, and smiles at her.
"Oh, were you unable to sleep as well, Tina?"
"H-hey Dan, it's m- ... huh?"
Before "Tina" can question either the fact that he immediately recognised her, or the fact that he called her Tina. He sits down next to her, and reaches his arm around her back, softly cupping one of her shapely breasts.
"Mmh? You're all wet.. Did something happen? Are you hurt?"
"No, I'm okay, but something strange happened! I saw- Oh okay, that's whats most important.."
Dan lets out a genuinely relieved sounding sigh right after interrupting her. He then continues.
"Thennn, this is a great chance right? The others are still asleep, you're already undressed too. So, shall we have sex?~"
